Practice Setting

• Outpatient preventative care clinics
• Adult patients only (ages 17+)
• Chronic disease focus: diabetes, hypertension, kidney care
• Behavioral health available via telehealth at all sites

Schedule

• Monday to Friday, 8a–5p (Cleburne site: 7:30a–5:30p with early Friday)
• Part-time or short-term commitments welcome
• Clinics staffed with one other provider and dedicated MA

Cases and Procedures

• Routine primary care visits
• Chronic disease management
• No procedures required

Requirements

• Board certified or eligible in Family Medicine or Internal Medicine
• Active Alabama license required (IMLC not accepted)
• Prior outpatient experience preferred
