Practice Setting

• Outpatient Health Center focused on preventative care.
• Provides care to client employees and their dependents.
• Work with dedicated RN support staff.

Schedule

• Start asap through March 2026.
• Consistent weekly schedule: Monday 7am-12pm then Tuesdays & Thursdays, 12pm-5PM.
• Short-term commitment without locking into months-long contracts.
• This schedule requires no meal break.

Cases

• Focus on episodic, preventive, and acute health care.

Requirements

• Must be Board Certified or Board Eligible in Family Medicine.
• Active Kentucky State Licensure is required
• BLS and DEA certifications are necessary.

Credentialing Requirements

• Credentialing in 7 days with a completed application.
• Requires a full drug screen and TB test for processing.
