Practice Overview
- You’ll join a 3-provider team: 1 supervising physician and 2 PAs, with experienced nursing support
- Outpatient primary care across two clinics (Buffalo and a second site about 20 minutes away), plus ER call coverage at the main hospital
- Broad-spectrum primary care with space for in-office procedures; identical clinic setups with MAs, nurses, and front-office support
- Epic EMR; payor mix approximately 60% Medicare, 30% commercial, remainder Medicaid/self-pay
Schedule
- Clinic hours: Monday–Friday, 8:00am–5:00pm; average ~14 patients/day
- Clinic split rotates between locations (typically 2 days at one site and 3 days at the other, alternating weeks)
- ER call only: typically 1–2 days/week and 1 weekend/month; schedule provided about 60 days in advance
- Built-in admin time: one day off every other week (can be taken from home)
Compensation & Benefits
- $150,000 - $160,000 guaranteed salary (hospital-employed) with a 3-year commitment
- $5,000 Retention bonus after 12-months and another $5,000 after 24-months
- $10,000 Relocation assistance
- $75,000 HRSA loan forgiveness (non-taxed, paid directly to lender) plus $5,000 relocation (with receipts per IRS guidelines)
- Time off and CME: 160 hours discretionary time off; 5 CME days with $4,000 annual allowance

Candidate Requirements
- Physician Assistant (NPs with acute care certification may also be considered)

- Comfortable managing outpatient primary care and participating in ER call coverage
- Current or ability to obtain BLS, ACLS, PALS, and ATLS
