Practice & Cases

• Outpatient primary care for adults primarily age 65 and older
• Manage diabetes, hypertension, hyperlipidemia, CHF, coronary artery disease, and mild cognitive impairment
• Nurse Practitioner, Medical Assistant or scribe, and Registered Nurse support
• No procedures required

Schedule

• Monday through Friday
• 8:00 a.m. to 5:00 p.m.
• Weekday-only clinic coverage
• Outpatient practice with no call

Requirements

• Must be an MD or DO
• Active Illinois medical license required
• Board Certified in Internal Medicine or Family Medicine
• DEA required and Spanish fluency required

• Mileage reimbursement provided
• Lodging, airfare, and rental car are not provided
