### Practice Overview

  • You’ll lead a small Memphis-based team of 2 APPs while managing a personal patient panel
  • Home-based care model with no clinic, hospital, or facility-based responsibilities
  • Manage chronic conditions for approximately 140 homebound patients
  • Serve as supervising physician, mentor APPs, lead care planning, and conduct ride-alongs

### Schedule

  • Monday–Friday only, with no weekends, nights, or after-hours call
  • Balanced model: 50% clinical visits and 50% administrative/leadership time
  • 6–8 in-home visits per day, each lasting 30–40 minutes
  • Flexible, mobile schedule supported by a dedicated care team

### Candidate Requirements

  • Board-certified or board-eligible in Internal Medicine, Family Medicine, or Geriatrics
  • Comfortable providing in-home care and mentoring APPs
  • Leadership mindset with interest in care coordination and quality improvement
  • Must be licensed or eligible for licensure in Tennessee
