Cases and Procedures

• Chronic care management: diabetes, CHF, CAD, HTN
• Mild cognitive impairment and complex care included
• No procedures required

Requirements

• Board Certified in Family or Internal Medicine
• Active Louisiana license and DEA required
• At least 1 year of experience
• Spanish fluency preferred
