Schedule

• Monday - Friday, 8 AM - 5 PM

Procedures and Cases

• Primary care for seniors
• Emphasis on preventative health

Requirements

• Board Certified in Family Medicine
• BLS certification
• Active Florida Medical License or IMLC Letter of Qualification
• Minimum of 12 months of experience

Credentialing Requirements

• Credentialing timeframe ranges from 30-120 days
• We will wait for your license with an active IMLC LOQ
